No one service can ever meet everyone’s needs and although there will always
be gaps in service provision we feel, through our various pathways, we are best equipped to meet the ongoing and changing support needs of our clients.
Our pathways are flexible and interchangeable so, while someone may initially be referred to supported housing designed for people with high needs, they will hopefully move into lower need supported housing and finally into outreach within the community.
In a similar way, someone may be a hospital link client but then need an ongoing outreach service such as floating support or active outreach.
Some services can be accessed concurrently so a person may, for example, be
a supported housing client but also have access to our health and wellbeing team.
