Admission Prevention supports you to stay well in your community.

Admission Prevention is a partnership between Julian Support and Norfolk & Suffolk NHST Foundation Trust (NSFT), who commission the service.

The service is designed for people who would be highly likely to need a hospital admission in the absence of additional support being available.

Our staff will help you to stay well in your community with the support of the people that matter to you.

We work between 8am and 8pm seven days per week to ensure that you can access the service when you need it. The team covers North, Central & South Norfolk and we can visit you in your own home or in a location that best suits you.

The support that we provide is holistic – we will focus on the things that are important to you and that will help you to remain in the community.

People can access the service for up to two weeks if they are living in the community, or six weeks if they have been discharged from hospital and there is concern that they may be readmitted if there is insufficient support in place for them.

We will work closely with NSFT, and other agencies involved in your care, throughout the period of time that you receive support from us.
