What is the aim of ARCH?

We support people with their mental health from crisis to home. Helping people stay in the community to prevent the need for a hospital admission or re-admission, working in collaboration with Norfolk & Suffolk Foundation Trust (NSFT) and others in the crisis pathway. 

We will work with you to make a plan around how we might be able to help. Focusing on short-term crisis intervention and having a strengths-focused, person-led approach.

  • Pre-Admission: The service can provide support for people who are ‘highly likely’ to be admitted to hospital without stepped-up support in the community.
  • Discharge Support: The service can support people to re-integrate into the community with the aim to prevent re-admission. The service can start working with people up to two weeks prior to their discharge.
     

 

When does the service operate?

We can work with you for up to 6-weeks and across all 7 days of the week.

Support will start from the assessment date and the support will be reviewed every 2-weeks.

Who is eligible to access this service?

People who:

  1. are 18+
  2. live in Norfolk & Waveney
  3. are under secondary mental health services 

 

 

How long can someone have support from ARCH?

The service offers one hour of daily support, depending on the needs of the person, for up to six weeks at a time.

Support is reviewed every two weeks and there is the option for people to bank hours if they do not require them all at once. 

A person's support is not expected to exceed six weeks within any six-month period.

 

 

What does this service offer?

Common examples of the support people can receive from ARCH:

  • Emotional support - give you a non-judgemental space for you to talk about how you are feeling
  • Go to groups and activities in your area with you
  • Point you in the direction for longer-term services to support with your housing or financial situation. 
  • Help you book and go to appointments with you e.g. GP or mental health
  • Help to get you into a routine with supporting and prompting activities of daily living e.g., cooking, cleaning, using public transport
  • Talk to other people you are working with

 

 

What is the referral process?

Referrals to ARCH can be made by CMHTs, CRHTs, Youth Teams, Ward Staff and Discharge Facilitators at NSFT.

Please note that, unfortunately, we are unable to respond to self-referrals or referrals from other agencies.

  2. Email a completed Referral Form and up-to-date Combined Assessment
  3. An assessment will begin within 48 hours
