The Pathway & Recovery Teams are located in West Ipswich and West Suffolk. They provide support to people in their own homes and other community settings. They also provide some in-reach to people living in Julian Support’s 24hr Supported Housing Services. The Pathway & Recovery Team in West Ipswich provides an in-reach service to the hospital, supporting people prior to and post discharge.

The service is available Monday to Friday between 9am and 5pm, and offers people a wide range of emotional and practical support.

REFERRALS:
Referrals to Pathway & Recovery Teams can be made by Norfolk & Suffolk NHS Foundation Trust (NSFT) through Adult Community Services at Suffolk County Council.
